Résumé

Abstract A scoping literature review to identify the multilevel HIV serosorting related elements was developed. Articles from EBSCO, PubMed, PsyNET and Science Direct with serosort* or serosorting at the tittle or abstract, written in English or Spanish were included. No restriction in type of population or design were applied. 239 records were retrieved after duplicates removed, but 181 references were extracted for full-text review. Individual level: HIV knowledge, serostatus, risk perceptions, abilities to disclose and for condom use negotiation, motivations, use of drugs, stigma, attitudes toward condom use, and perceptions/beliefs about the HIV and related treatments, HIV infection rates/testing and behavioral factors. Interpersonal level: social networks, abilities (sexual behavior negotiation, and communication). Community level: stigma, social norms, access to HIV related services. Structural level: political context, HIV related funding and public policies. HIV Serosorting is not solely an interpersonal behavior it involves multilevel elements that must be acknowledged by professionals and stakeholders.


Resumen Se desarrolló una revisión de alcance de la literatura para identificar elementos multinivel relacionados a la seroclasificación de VIH. Se incluyeron artículos de EBSCO, PubMed y Science Direct con serosort* o serosorting en título o resumen, escritos en Inglés o Español. No se aplicaron restricciones por tipo de población y diseño. Después de remover duplicados, se recuperaron 239 records, solo 181 referencias se extrajeron para revisión a texto completo. Nivel individual: Conocimiento del VIH, seroestado, percepciones de riesgo, habilidades para develar el seroestado y negociar el condón, motivaciones, uso de drogas, estigma, actitudes sobre uso del condón, y percepciones/ creencias acerca del VIH y tratamientos, tasas de infección y tamizaje de VIH, factores conductuales. Nivel interpersonal: redes sociales, habilidades (negociación de la conducta sexual, y comunicación). Nivel comunitario: Estigma, normas sociales, acceso a servicios de VIH. Nivel estructural: contexto político, políticas públicas y financiamiento relacionado al VIH. La seroclasificación de VIH no es solamente una conducta interpersonal, incluye elementos multinivel que deben ser reconocidos por los profesionales de salud y tomadores de decisiones.

Background: HIV infected children survive to adolescence because of anti retroviral therapy, however, only a small proportion know their diagnosis.Disclosure is critical to long-term disease management, yet little is known about if, how, and when disclosure takes place and the barriers associated with it, and its impact on children in resource limited settings. Objective: This study set out to determine the process of and barriers to HIV disclosure in children as well as the immediate impact of this on children and their caregivers. Methods: A cross-sectional study was done June-July 2016 using a structured questionnaire, convenience sampling and quantitative methods at the infectious disease clinics of National Hospital Abuja. A sample of 164 caregivers of HIV positive children aged 5 to 16 years receiving antiretroviral therapy for at least one year were enrolled. Results: Prevalence of full disclosure was 24.5%, partial 22.7% with overall prevalence of 47.2%. Main barrier to disclosure was child's age and fear of informing others. The impact of disclosure on caregivers was relief in 45.5% but emotional and difficult for others. Immediate reactions by children were sadness; tearfulness and worry in 28.6%, some showed no reaction while others even expressed relief. On a longer term, disclosure had several effects. Main predictors of disclosure on regression were the child's age and caregiver's opinion on disclosure. Conclusion: The prevalence of full disclosure is low and several barriers affect disclosure. Caregiver's and HCWs need empowerment and support with culturally appropriate skills and platforms to deal with the barriers, process and impact of disclosure

Résumé

The introduction of antiretroviral medication has changed the epidemiology, morbidity and mortality of HIV disease. Nevertheless,cutaneous disorders due to HIV infection remain a major problem in HIV-infected patients. These disorders are mainly infections,inflammatory and neoplastic in origin. Some of these disorders occur at normal CD4 cell counts, while others occur typically at low CD4 cell counts. Clinicians should be aware of various presentations of these disorders and their treatment as they can impact negatively on patient's quality of life

Human immunodeficiency virus (HIV) infection not only leads to a compromised immune system, but also disrupts normal haematopoiesis, resulting in the frequent manifestation of cytopenias (anaemia, thrombocytopenia and neutropenia). Although there is a definite association between the severity of cytopenia and HIV disease stage, this relationship is not always linear. For example, cytopenias such as thrombocytopenia may occur during early stages of infection. The aetiology of these haematological abnormalities is complex and multifactorial, including drug-induced impaired haematopoiesis, bone marrow suppression due to infiltration of infectious agents or malignant cells, HIV-induced impaired haematopoiesis, and several other factors. In this review, we describe the frequencies of anaemia, thrombocytopenia and neutropenia reported for HIV-infected, treatment-naïve cohorts studied in eastern and southern sub-Saharan African countries. We present a rational approach for the use of diagnostic tests during the workup of HIV-infected patients presenting with cytopenia, and discuss how HIV impacts on haematopoietic stem/progenitor cells (HSPCs) resulting in impaired haematopoiesis. Finally, we describe the direct and indirect effects of HIV on HSPCs which result in defective haematopoiesis leading to cytopenias

Résumé

This article examines the reality of HIV-positive women being subjected to perpetual stigmatisation as a result of involuntary sterilisation practices. The reproductive autonomy and dignity of HIV-positive persons is protected by various constitutional provisions, and a legal framework providing for the requirement of informed consent, as well as the prohibition of discrimination on the grounds of HIV status. This article considers the issues of both informed consent and discrimination in the context of the practice of involuntary sterilisation of HIVpositive women. The article considers the legal framework in light of the physical, emotional, social and cultural implications for HIV-positive women who are subjected to involuntary sterilisation

Résumé

Understanding the social conditions and immunological characteristics that allow some human immunodeficiency virus (HIV)-exposed patients to remain uninfected represents an on-going challenge. In this study, the socio-demographic and sexual behaviour characteristics and immune activation profiles of uninfected individuals exposed to HIV-infected partners were investigated. A confidential and detailed questionnaire was administered and venous blood was tested using HIV-1/enzyme immunoassays, plasma HIV-1 RNA levels/bDNA and immunophenotyping/flow cytometry to determine the frequencies of CD4 and CD8 T cells expressing activation markers. The data analysis showed significant differences (p < 0.05) for immune parameters in individuals who were uninfected, albeit exposed to HIV-infected partners, compared with unexposed individuals. In particular, the exposed, uninfected individuals had a higher frequency (median, minimum-maximum) of CD4+HLA-DR+ (4.2, 1.8-6.1), CD8+HLA-DR+ (4.6, 0.9-13.7), CD4+CD45RO+ (27.5, 14.2-46.6), CD4+CD45RO+CD62L+ (46.7, 33.9-67.1), CD8+CD45RA+HLA-DR+ (12.1, 3.4-35.8) and CD8+CD45RO+HLA-DR+ (9.0, 3.2-14.8) cells, a decreased percentage of CD8+CD28+ cells (11.7, 4.5-24.0) and a lower cell-surface expression of Fcγ-R/CD16 on monocytes (56.5, 22.0-130.0). The plasma HIV-1 RNA levels demonstrated detectable RNA virus loads in 57% of the HIV-1+ female partners. These findings demonstrate an activation profile in both CD4 and CD8 peripheral T cells from HIV-1 exposed seronegative individuals of serodiscordant couples from a referral centre in Belo Horizonte, state of Minas Gerais.

Résumé

Refletindo sobre homossexualidade, Aids e seus desdobramentos sociais e subjetivos nos últimos 30 anos, procuramos nesta dissertação discutir o fenômeno bareback – sexo sem camisinha - nomeado nos Estados Unidos na segunda metade da década de 1990. Sua disseminação na mídia tem causado, com frequência, reações que reconectam a homossexualidade a loucura, doença e morte. Devido à restrita produção acadêmica no Brasil, objetivamos contribuir por meio deste trabalho com algumas considerações essenciais ao debate. Percorremos alguns deslocamentos historicamente importantes relativos à homossexualidade, a condução das condutas - práticas de governo, risco, Aids e ao próprio bareback. Neste sentido, o trabalho associa um estudo teórico sobre este objeto a entrevistas realizadas na cidade do Rio de Janeiro. A pesquisa empírica exploratoria recolheu dados e discursos sobre este fenômeno em nossa realidade e contexto, tendo como terreno a Associação Brasileira Interdisciplinar de Aids (ABIA) e o Grupo Pela Vidda-RJ, duas organizações não-governamentais que trabalham com a Aids, e no Grupo Arco-íris, ONG integrante do Movimento LGBT. Duas pessoas de cada uma destas ONGs foram entrevistadas. Buscamos entender como essas instituições, locais privilegiados de nossa incursão, vêm abordando o fenômeno, quais suas posições e impressões. Paralelamente, contactamos alguns voluntários adeptos do sexo bareback, por considerarmos seus discursos indispensáveis e capazes de tornar este trabalho mais rico e diverso, no entendimento do bareback, a partir de suas experiências individuais. Para tal, utilizamos dois sites de bareback internacionais (barebackrt.com e bareback.com) que hospedam perfis de brasileiros, alguns residentes na cidade do Rio de Janeiro...


Reflecting on homosexuality, AIDS and its social and subjective outcomes in the last 30 years, we discuss the bareback phenomenon - sex without a condom - as named in the United States in the second half of the 1990s. Its dissemination through the media has often caused reactions that reconnect homosexuality with madness, disease and death. Due to the limited academic production in Brazil, we aim to contribute through this work with some essential considerations for the debate. We will cross some historically important shifts relating to homosexuality, the conducts – governance practices, risk, AIDS, and bareback itself. In this sense, this work associates a theoretical study on this subject with interviews conducted in the city of Rio de Janeiro. The exploratory empirical research gathered data and reports on this phenomenon in our reality and context, in the Brazilian Interdisciplinary AIDS Association (ABIA) and Grupo Pela Vidda-RJ, two nongovernmental organizations that work with the AIDS issue, and the Grupo Arco-Íris, an NGO member of the LGBT movement. Two people from each of these NGOs were interviewed. We tried to understand how these institutions, privileged place in our survey, have approached this phenomenon and what are their positions and viewpoints. Concomitantly, we contacted some volunteers supporters of bareback sex, as we consider their speeches essential, and able to make this work rich, consistent and multilateral, concerning bareback, based on their individual experience. To this end, we accessed two international bareback sites (barebackrt.com and bareback.com) that host profiles of Brazilians, including some residents in the city of Rio de Janeiro...
